Welcome to City Ophthalmology. We are excited to have you join us. For your first visit, we request that you bring the following documents for registration:

Photo Identification (e.g. Driver's Licence, Passport, etc)

Insurance Card (please know the member's name, date of birth, and address)

DILATED EYE EXAM
For your first visit, or a follow-up examination after a year or more, you will likely undergo a dilated eye exam. Dilation is an important part of a comprehensive eye exam because it enables your eye care professional to view the inside of the eye. Drops placed in each eye widen the pupil, which is the opening in the center of the iris (the colored part of the eye).
You should be aware that a dilated eye exam will take longer and your vision may be blurry temporarily for several hours, and you may be more sensitive to light after the examination. Disposable sunglasses will be made available to you, but please make arrangements for transportation home if you are uncomfortable driving after a dilated exam.
